EPDS Screening: Using the Edinburgh Scale Well in Perinatal Care

The Edinburgh Postnatal Depression Scale is the global standard for perinatal mood screening: 10 items about the past 7 days, several reverse-anchored, total 0-30, free to reproduce with citation.

Bands

• 0-9 Low likelihood of depression
• 10-12 Possible depression — re-screen in 2 to 4 weeks, supportive follow-up
• 13+ Probable depression — full clinical assessment and active planning

Item 10 comes first

Item 10 asks about self-harm thoughts. Any endorsement gets direct, same-day review regardless of total score, the EPDS total can be modest while item 10 signals real risk.

Cadence, not one-and-done

Perinatal mood shifts fast. Screen at least once antenatally and at 6 to 8 weeks postpartum, and re-screen whenever the clinical picture changes. A single negative screen at week 2 says little about month 4.

Beyond the birthing parent

The EPDS validates acceptably for fathers and non-birthing partners (often with slightly lower cut-offs discussed in the literature, around 10). Screening both parents when one is struggling is increasingly standard.

What the EPDS is not

It's not a diagnostic instrument, not an anxiety measure (though items 3 to 5 lean anxious), and not a substitute for asking about intrusive thoughts, bonding, and support directly.
